Wellness clinic for the 2026 enrollment period

- Reminder - All members and their spouses are required to have a mandatory wellness visit 

All requirements must be dated in the 2025 Calendar Year! 

Clinic will include : 

* Wellness Clinic - Walk - in

* Hearing Screening - By appointment only - Please call Mary at 401-943-3033 ext 221

* Colonial Life Representative (Gordy Jones)

Both the member and their spouse must complete the Wellness Program to qualify for the Colonial Life Bridge Policy ( Family Plan)
